Our At Home Healthcare Services

  • Home Health Care: Short-term skilled nursing and therapy to help you recover from surgery, a severe illness, or treatment for a medical condition. Our professional team includes skilled nurses, physical, occupational, and speech therapists; social workers and home health aides.
  • Palliative Care: Support for people with a complex or chronic disease. We provide services to address the physical, emotional, and spiritual needs of patients, families, and caregivers to help achieve the highest quality of life possible.
  • Hospice Care: Medical care, pain management, and emotional-spiritual support for patients at the end of life and their families.We provide care in the patient's home nursing homes or skilled nursing facilities
  • Personal Care: Helps people remain independent by helping with meal preparation, daily living activities, and household help while providing companionship to the patient and respite for caregivers.
  • Adult Day Services: Located in St. Marys, our center provides daytime supervision for adults who cannot remain at home alone while giving respite to caregivers.

What type of professionals provide at home healthcare?

We offer a wide range of home healthcare services provided by specially trained staff. Depending on the services you will receive, you may be cared for by: The Penn Highlands Healthcare at Home team includes all levels of in-home health professionals. Depending on your needs, your home healthcare team may include:

  • Home Health Services are provided by trained and licensed medical professionals including registered nurses, personal care aides, physical therapists, occupational therapists, speech therapists, social workers, and nutritionists.
  • Hospice Care is provided by a team coordinated by your healthcare provider and overseen by our hospice medical director and a pharmacist. Your in-home team may include a nurse, aides, social workers, pastoral care, and bereavement counselors. Trained and verified volunteers also provide service, if requested by the family.
  • Palliative Care nurses provide pain and symptom management to patients and coordinate other forms of care with therapists, aides, clergy, and social workers.
  • Personal Care Services are provided by our personal care aides, and although these services do not require a licensed professional or a physician's order, some services will be provided by professional nurses.
  • Adult Day Services are provided in our adult day center in St. Marys by professionally trained personnel experienced in working with seniors, including those with dementia, Alzheimer’s disease, or a physical disability.
Will my insurance pay for home healthcare?

Many insurance plans, as well as Medicare and Medicaid, will pay for some certified home health services if ordered by your physician. We are a certified home health service. There may be some limitations, or you may have to pay a portion depending on your insurance plan. Personal care is typically not covered by insurance. We will help you understand any costs to you before beginning care.

How do I get home healthcare?

The best way to obtain home health nursing and other home healthcare services is to discuss your needs with your physician. If your physician believes you will benefit from care by a home health nurse or other home care service, they can authorize a treatment plan, commonly called a plan of care, and make a referral to us. Can I call to set up home healthcare?

Yes, our team can help you understand the different levels of care and which might be the most appropriate for your loved one. If you need home healthcare, palliative care, or hospice care, we will contact your doctor for approval to evaluate your situation and to get necessary medical information. What information will I need to set up at home healthcare services?

When you or a family member calls Penn Highlands Healthcare at Home, you will be asked basic information, such as your name, date of birth, address, and phone number. We also will need information about your doctors, diagnosis, current medical orders, and any prior hospitalizations. Safety is very important. There may be questions about your medications, whether you have had a flu shot, concerns about personal safety in your home, and who your caregiver and/or emergency contact will be. We are happy to provide you with a list of information we will need if you call us.
