Telehealth Services
The Penn Highlands Community Nurses use telehealth units in many homes to supplement the care of patients who have certain medical conditions that require close monitoring of symptoms. A patient monitor is placed in the patient’s home. The patient follows simple instructions to take vital signs, enters information and then pushes a button to transfer all the information back to a central monitor station located in our office.
Our Telehealth Registered Nurses closely monitor incoming information, assesses it and immediately addresses any concerns with the patient and his or her doctor. Telehealth monitoring helps reduce emergency room visits and hospitalizations by alerting our nurses to potential problems and intervening in a timely manner.
Telehealth monitoring is particularly helpful for patients with chronic conditions such as chronic obstructive pulmonary disease, congestive heart failure, diabetes and asthma.
Electronic Medication Dispenser
Helping patients avoid medication errors is a top priority at Penn Highlands Community Nurses. A medication error from overdosing, missing doses, addiction, to forgetting to refill prescriptions, is a serious health concern among our senior citizens. Electronic medication dispensers are available for our patients who have difficulty managing their medications. MedSmart is a programmable, locked pill dispenser for dispensing pills, reminding the user when medicine should be taken, and making the correct dose available. Once the patient takes his current dose, the unit automatically resets for his next reminder/alarm time.
Lifeline Medical Alert
Lifeline Medical Alert is an easy-to-use medical alert service designed to reduce the risk of living alone. In the event of a fall or emergency, help is available at the push of a button. The Lifeline Personal Help Button connects you to a trained Personal Response Associate who can send help quickly.
