At Penn Highlands Healthcare, we understand there are times when a patient no longer needs the acute care they receive in a hospital, yet they may not feel strong enough to return home. For those patients, we offer a comforting alternative. Our Swing Bed units are available at all five Penn Highlands hospitals in Connellsville, Brookville, Huntingdon, St. Marys and Tyrone for patients near discharge who need to regain their strength and mobility after an illness, injury or surgery.

Recovering from a serious event, such as a stroke, or a major surgery can be overwhelming for patients. The Swing Bed Program at Penn Highlands is a great option for those who may not have the confidence or support to leave the hospital and take care of themselves.

Who could Benefit from the Swing Bed Program?

  • Any patient needing to regain strength and mobility after an illness, surgery or injury.
  • Any patient that requires IV antibiotic therapy for an infection or daily complex wound care.

A plan of care will be developed with you, your family and a team of professionals to assist in meeting your goals. Your health care team will include:

How Long will My Stay be in Swing Bed?

The length of your stay will be based on your progress to meet your therapy goals as well addressing your individual medical needs. Your progress will be assessed regularly and discussed with you by members of your health care team.

Your case manager or social worker will assist you in planning for your discharge and help set up any services or equipment you may need at home. This will allow you to have a smooth transition to your doorstep.

Swing Bed Care Helps Make The Transition To Home

Through a combination of first-rate rehabilitative therapies, attentive nursing care and medical supervision, patients gain the strength, functionality, balance and range of motion they need to care for themselves with confidence.

Medicare and most insurance companies cover post-acute rehabilitation services. A patient with Medicare insurance must be hospitalized as an “acute care inpatient” for a minimum of three consecutive midnights within a 30-day period and most commercial insurances can provide authorization for swing bed services. Should a patient be discharged home after an inpatient admission and realize that he or she does need additional support, the patient may be considered for a Swing Bed stay if qualified. Primary care physicians, emergency departments and home health care can recommend and provide information and consideration for a Swing Bed admission. Swing Bed admissions can come from any hospital.

Continue On The Road To Recovery

After discharge from the Swing Bed program, patients can continue their rehabilitation at one of Penn Highland’s 25+ outpatient rehabilitation clinics or through home health offered by Penn Highlands Healthcare at Home.
