Home Care Agencies Share Best Practices

Joyce discussed the process used at the Community Nurses’ weekly interdisciplinary meetings where staff members use a team approach to develop a plan of care for each patient to achieve desired goals and outcomes. The discussion that followed provided valuable information for each agency on the different methods and procedures currently in use.
